CASE MANAGEMENT

Our Case Management Programs Division places full-time site coordinators directly within Duval County schools, where they provide support for students who are at risk of falling behind or dropping out. The programs in this division provide access to many important services, including individual and group-session counseling, home visits, and coordinated parental involvement.

THE STUDENT ENRICHMENT PROGRAM

The Student Enrichment Program is the original program that Communities In Schools began in Jacksonville schools more than 25 years ago. The program places full-time site coordinators on school campuses throughout Duval County, where they work individually with 90 to 100 students. These students are counseled individually and in group sessions throughout the year, enabling them to realize their goals and providing them with the life skills they need to succeed. Home visits and parental involvement are essential parts of the program, and site coordinators connect students to needed community resources.

While site coordinators work one-on-one with select students from the school’s student body, hundreds of others are reached through school-wide services such as career fairs, men’s and ladies’ seminars and literacy programs. Funding for the Student Enrichment Program is provided by Duval County Public Schools, The Jim Moran Foundation, the DuBow Family Foundation, Boeing, The Coach Foundation, The Beaches Community Fund, TPC Red Coat Charity Committee, The Edna Sproull Williams Foundation, The Community Foundation for Northeast Florida, and the Arthur Vining Davis Foundation.

GEAR UP

Communities In Schools is a partner with Duval County Public Schools in two 7-year Federal GEAR UP grants. The GEAR UP programs start with 7th grade cohorts in 10 DCPS middle schools. The grant is cohort-based and will follow these students through high school and one year of post-graduation transition. Communities In Schools case manages groups of students at each participating school.

ACHIEVERS FOR LIFE (AFL)

Achievers For Life (AFL) is a dropout prevention initiative developed and funded by United Way of Northeast Florida, in partnership with Communities In Schools of Jacksonville, Jewish Family and Community Services, and Big Brothers Big Sisters of Northeast Florida. Achievers For Life targets sixth grade students exhibiting risk factors of school disengagement including low GPAs, low FCAT scores and excessive absences and provides them with family support, a mentor and help for the students at school. Communities In Schools achievement advocates work with students to keep them on track and prepare for high school.

BRIDGE TO SUCCESS

Funded by Duval County Public School, the Bridge to Success Program provides site coordinators who work with students currently over-age for their grade level. Site coordinators address all barriers to graduation and provide individuals with the support to help them graduate on time and ready to fulfill their college and career goals.
